Modifications To An Existing, Approved Protocol

The request should contain:

1. A brief description of and explanation for the revision(s) or modification(s). Describe the implications on your research design, recruitment process, process for obtaining informed consent, anonymity or privacy.

2. A copy of the revised/new questionnaire, survey instrument, interview protocol with the revised/added section(s) highlighted.

3. A copy of the revised consent form, information letter, recruitment poster, with the revised/added section(s) highlighted that are impacted by the revision(s).
